NOTICE: The first stats submission of the year needs to be entered on your profile page manually. Please allow 24 hours for your 2024 stats to appear on your profile page before contacting TLC HQ. Thank you!

How to enter stats:
1. Select the month you are reporting. Update year if necessary.
2. Enter submitter name (required).
3. Enter your stats (do NOT use commas, $ signs or decimal points). Round to the nearest dollar (do not add change/cents).
4. Click "Submit"
Doctor's Name

Practice Name
Please enter the practice name for the stats you are submitting.
Practice name is pre-filled with your primary practice. Please update if submitting stats for an additional practice.
Doctor's Email
Please enter the email address where you would like to receive the results of your submission.
